New Study Finds Childhood Maltreatment Increases The Risk Of Intimate Partner Violence

A Lancet study explored why childhood maltreatment increases the risk of intimate partner violence (IPV) victimization. Using data from over 11,000 twins, researchers rigorously assessed personality and mental health as mediators, controlling for genetic and environmental factors.

The study found that childhood maltreatment independently shaped psychological development, explaining 65% of the link to IPV. Key pathways included low subjective wellbeing, conduct problems, and aggression. Risk-taking, often cited, was not a significant mediator in their stringent analysis.

These findings underscore the importance of supporting mental health and wellbeing in young people who have experienced maltreatment, particularly by targeting these psychological vulnerabilities and relationship navigation skills to disrupt the cycle of violence.

A new study published in The Lancet looked at personality and mental health as mediators linking childhood maltreatment to intimate partner violence victimization. 

“We know from previous research that people who experience abuse or neglect in childhood are much more likely to be victimized by a romantic partner later in life,” study author, Patrizia Pezzoli told us. “That is a robust finding, but what we don’t fully understand is why. What is it about childhood maltreatment that increases that risk?” 

The study set out to identify the specific psychological pathways connecting those two experiences, and to do so more rigorously than had been done before. In particular, the research team wanted to know whether personality and mental health difficulties transmit the effect of childhood maltreatment on later relationship harm, or whether they are simply markers of a broader, pre-existing vulnerability.

“We did have some expectations going in,” Pezzoli told us. “Based on the existing literature, we predicted that personality and mental health traits would partly explain the link between childhood maltreatment and intimate partner violence victimization.”

The researchers also expected to find shared genetic and environmental influences running underneath it all. This means that some of the association reflects the fact that there are risk factors, genetic and environmental, that can make people more likely to both develop certain traits and have certain experiences. 

„What we were not sure about was which specific traits would hold up as genuine pathways once we took into account those underlying risks,” Pezzoli told us. “Intimate partner violence, often called domestic abuse in the UK, is a major public health problem, and people who experience childhood maltreatment are especially vulnerable to this experience.”

Pezzoli explained that if we can better understand why, we have a much better chance of intervening and disrupting that cycle. There is also something important about doing with really robust methods. Previous studies have often looked at one or two traits in isolation, in relatively small samples, without the tools to distinguish true mechanisms from background noise. This research team felt there was a real opportunity to do something more rigorous and, hopefully, more useful for prevention.

“We used data from over 11,000 participants of the Twins Early Development Study, which is a large UK cohort of twins born in the 1990s, who completed measures of 18 different personality and mental health characteristics at age 22, alongside questions about their childhood experiences and any abuse within their adult relationships,” Pezzoli told us. “Studying twins is powerful because it lets you tease apart what is shared within families (genes and shared environments) from what is unique to each individual. We also used a causal inference method called Mendelian Randomization Direction of Causation, which allowed us to test whether the psychological traits were genuinely transmitting the effect of maltreatment, over and above shared underlying vulnerabilities.”

The study revealed that even after accounting for inherited and shared environmental vulnerabilities, childhood maltreatment still independently shaped psychological development in ways that increased the risk of intimate partner violence. In fact, around 65% of the total effect of maltreatment on later victimization operated through the psychological pathways that were measured. 

Once those psychological pathways were accounted for, no statistically significant direct effect of maltreatment on victimization remained, which suggests that the psychological impact of maltreatment is really driving the association. Three traits stood out as particularly important: low subjective wellbeing, conduct problems, and aggression, which could be promising targets for interventions aimed at promoting safe intimate relationships following maltreatment.

“We were perhaps most struck by how much of the association was explained through psychological pathways, as 65% is substantial,” Pezzoli told us. “What also caught our attention was what didn’t hold up. Risk-taking, for example, was as a significant mediator in traditional, less stringent models, but not in the most rigorous analyses.” 

That matters, Pezzoli explained, because risk-taking, particularly sexual risk-taking, has often been emphasized in the literature as a key mechanism increasing likelihood of relationship harm, which might be misinterpreted as suggesting some kind of victim responsibility for their own victimization. Understanding that risk-taking is itself shaped by deep genetic and environmental factors can help shift the narrative in a more constructive direction.

“The results reinforce just how important it is to support the mental health and wellbeing of young people who have experienced maltreatment, also because doing so may help protect them in their relationships,” Pezzoli told us. “The fact that we identified key psychological pathways gives us something concrete to keep in mind when designing preventative measures.” 

The findings also suggest that interventions for this group may be particularly beneficial if they included elements specifically focused on navigating relationships, helping young people understand and manage the ways that early adversity can shape how they relate to others. 

“Adolescence seems a particularly important window for this, when relationship experiences are still being shaped and there is great potential to interrupt harmful patterns,” Pezzoli told us. “One thing we feel strongly about is how these findings should not be read. The fact that childhood maltreatment can shape personality and mental health in ways that increase risk does not mean that individuals are in any way responsible for their victimization. These psychological vulnerabilities can emerge as adaptations to very difficult circumstances.” 

Pezzoli also believes that the genetic findings can be counterintuitive. Discovering that experiences like childhood maltreatment and intimate partner violence are partly influenced by genetic factors does not mean that some people are genetically destined to be victimized, or that there is something fundamentally different about them. 

“We all carry some degree of genetic risk for less desirable traits and experiences, that is part of being human,” Pezzoli told us. “But this genetic risk is probabilistic, not deterministic, and it operates in interaction with the environment. Ultimately, what our findings offer is a better understanding of the pathways involved, so we can intervene earlier and more effectively for people who need it most.”
